We offer the following programs priced as a package of services, or Case Rates, so you know what to expect financially before you begin your treatment plan. Below are Case Rate options you might consider over the spectrum of care from the diagnosis to treatment of infertility.

Patient Assessment
If you do not have insurance or have insurance but lack coverage for the diagnosis of infertility services and are concerned about the cost of the initial patient assessment, Long Island IVF has developed a Patient Assessment Case Rate. This Case Rate includes the procedures necessary to diagnose the causes of infertility and to develop a treatment plan specifically for you. Eligibility for this Case Rate is determined when our financial counselor verifies benefits with your insurance plan and it is determined that you do not have coverage for the services included in the Case Rate.
The $500 Case Rate includes the following services:
- New patient office visit
- Baseline endocrine testing (Day 3)
- Water sonogram and ultrasound
- Semen analysis
IUI
Clomid IUI Case-Rate
If you do not have insurance or have insurance but lack coverage for Intrauterine Insemination (IUI) and are concerned about the cost of this procedure, Long Island IVF has developed a Clomid IUI Case Rate. This Case Rate includes the procedures in your Clomid IUI treatment plan specifically for you. Eligibility for this Case Rate is determined when our financial counselor verifies benefits with your insurance plan and it is determined that you do not have IUI coverage for the services included in the Case Rate.
The $1,200 Case Rate includes the following services:- Endocrine testing
- Ultrasound monitoring
- Cycle monitoring
- Semen analysis
- Intrauterine insemination
- Sperm preparation
The cost of drugs is not included in the IUI Case Rate.
Gonadotropin IUI Case-Rate
If you do not have insurance or have insurance but lack coverage for Intrauterine Insemination (IUI) and are concerned about the cost of this procedure, Long Island IVF has developed a Gonadotropin IUI Case Rate. This Case Rate includes the procedures in your Gonadotropin IUI treatment plan specifically for you. Eligibility for this Case Rate is determined when our financial counselor verifies benefits with your insurance plan and it is determined that you do not have IUI coverage for the services included in the Case Rate.
The $3,700 Case Rate includes the following services:- Endocrine testing
- Ultrasound monitoring
- Cycle monitoring
- Semen analysis
- Intrauterine insemination
- Sperm preparation
Micro IVF
If you do not have insurance or have insurance, but lack coverage for infertility services and are concerned about the cost of an IVF cycle, you might consider Micro IVF (sometimes called Minimal Stimulation IVF or Mini IVF) . We offer an initial consultation at no cost that includes an office visit with the physician. If IVF is determined to be the best course of treatment, we offer a Patient Assessment Case Rate of $500. After your Patient Assessment is completed, a treatment plan will be developed by our team of clinicians. Micro IVF might be the best approach for you when considering the cost of care between an IUI cycle and a fresh IVF cycle. Each patient’s case is considered carefully and individually.
Some causes of infertility which will not respond to IUI but are treatable with Micro IVF include:- Pelvic adhesions/scarring
- Blocked fallopian tubes
- Endometriosis
- Severe male factor issues
- Endocrine testing
- Ultrasound monitoring
- Cycle monitoring
- Egg retrieval
- Embryo transfer
- Nurse/Social worker visit
- Embryology procedures as per protocols which may include assisted hatching, co-culture, extended culture, embryo glue
- Suture, as per protocols
- Facility fee
The Micro IVF Case Rate is $ 3,900. This fee does not include medications and covers only the IVF cycle up to the diagnosis of pregnancy. Pregnancy monitoring is the responsibility of the patient. If you have insurance coverage, we will bill your insurance carrier. If you have no insurance coverage, this will be your financial responsibility.
There are a few procedures that clinically you may require or consider that are not included and must be paid prior to the IVF cycle start:- Anesthesia - $500
- ICSI - $2,000
- Cryopreservation of embryos and up to 6 months of storage - $1,155.
Frozen Embryo Transfer Following Fresh IVF Single Embryo Transfer (SET)
(Offered with the IVF Grant or Micro IVF)
If your previous fresh IVF cycle attempt with a single embryo transfer does not result in a clinical pregnancy and you have embryos cryopreserved, you can receive up to three (3) subsequent Frozen Embryo Transfers (sometimes called Post-Cryo Transfers) for one fee until a live birth is achieved. All attempts must be performed within one year of the first fresh IVF cycle.
Long Island IVF offers up three (3) of these Frozen Embryo Transfer cycles at a Case Rate of $4,425. These cycles include:- Endocrine testing
- Ultrasound monitoring
- Preparation of embryos for transfer
- Embryology procedures as per protocols which may include the thaw of cryopreserved embryo(s), culture of oocyte(s) less than 4 days, assisted hatching, embryo glue
- Embryo transfer
- Ultrasound guidance for transfer
- Suture, as per protocols
This Frozen Embryo Transfer Case Rate following a SET IVF cycle of $ 4,425 does not include medications and covers only the Frozen Embryo Transfer cycles up to the diagnosis of pregnancy. Pregnancy monitoring is the responsibility of the patient. If you have insurance coverage, we will bill your insurance carrier. If you have no insurance coverage, this will be your financial responsibility.
