NYS Financial Support for IVF

Long Island IVF has been selected by the New York State Department of Health to be a provider in their Infertility Demonstration Program. (Provider selection was based on published data on infertility providers in NYS (CDC's Assisted Reproductive Technology Success Rates).) This program provides financial support to insured patients without coverage for In Vitro Fertilization (IVF). Long Island IVF offers three convenient locations in Port Jefferson, Lake Success and also Melville, where our new state of the art IVF center is located. You might be eligible for the program if you are:

  • privately insured and your insurance coverage of these procedures is not a covered benefit
  • no longer covered for infertility services because your benefits are exhausted
  • a resident of New York State
  • between the ages of 21-44
  • clinically infertile and meet other health criteria.

Infertility services will be paid by New York State after reimbursement of any insurance coverage available, and after a cost sharing amount that the patients will be required to pay. Long Island IVF will assist eligible patients in identifying their portion of the cost share amount as the amount varies by the patient's household income and the cost of the procedures. The program was designed so that the patient's cost share cannot exceed ten percent (10%) of the patient's gross household income annually; therefore, financial support is available on a sliding scale basis.
